Work Environment
This position is based in a home care setting, allowing for a flexible, compassionate approach to rehabilitation. You will collaborate closely with a multi-disciplinary team including physicians, physical therapy assistants, and other healthcare providers. Key Responsibilities
  • Conduct comprehensive assessments of patients' physical and functional abilities to create tailored treatment plans.
  • Utilize a variety of therapeutic techniques, including advanced manual therapy, therapeutic exercises, and neuromuscular re-education, to enhance recovery.
  • Document patient progress meticulously and adjust treatment protocols based on evolving needs and recovery trajectories.
  • Educate patients and their families on injury prevention strategies, home-based exercises, and techniques for maintaining long-term wellness.
  • Maintain effective communication with all stakeholders, including families and interdisciplinary team members, to ensure seamless patient care.
  • Monitor and report any significant changes in patient conditions to the referring physician and provide recommendations for further treatment.
  • Participate in team meetings to discuss cases, share insights, and collaborate on best practices.
Patient Population / Client Focus
You will work primarily with patients suffering from orthopedic conditions, including but not limited to sports-related injuries, post-operative care, and chronic pain management. The home care setting offers a unique opportunity to address barriers certain patients may face in accessing traditional clinic-based therapy.
Qualifications
  • A valid New Jersey Physical Therapist license is required.
  • A Doctorate (DPT) or Master's Degree in Physical Therapy is essential for this role.
  • A minimum of 2 years of experience in outpatient orthopedics is preferred; however, new graduates with robust clinical experience are encouraged to apply.
  • Strong clinical assessment skills, empathetic communication abilities, and adaptability are critical for success.
  • Current CPR and First Aid certifications are preferred; additional certifications in specialized areas such as geriatrics or neurological rehabilitation will be considered a plus.
  • A valid driver's license and reliable transportation are essential, as travel to patients' homes is a requirement of this role.
Preferred Experience
  • Previous experience in a home care setting is advantageous, enhancing your ability to deliver individualized patient care.
  • Hands-on experience with diverse patient populations will be beneficial.
  • Familiarity with electronic health record (EHR) systems for documentation can significantly streamline workflow.
Compensation and Benefits
  • Competitive salary range of $90,000 - $100,000 per year, commensurate with experience and qualifications.
