Core Responsibilities and Essential Functions:
Patient Assessment:

Collaborate with physicians to develop, evaluate, and update treatment programs, care plans, goals, and discharge plans based on the patient's physical therapy needs.

Create and adjust care plans with input from the patient and/or family.

Develop realistic discharge plans incorporating patient and family goals.
Treatment Planning and Provision of Care:
-
Implement and adjust physical therapy treatment plans.
-
Select appropriate activities to help patients achieve their goals.
-
Revise goals and plans with input from patients and families.
-
Provide ongoing education and training to patients and families.
-
Give discharge instructions and referrals to community resources as needed.
-
Serve as a resource for staff, families, and physicians regarding rehabilitation services and equipment.
Documentation:

Document evaluations, treatment plans, and each session according to departmental guidelines.

Complete and submit all required documentation, including family education and discharge information.
